EXTENSION OF SHORT VALIDITY PASSPORT (EV)

       In certain urgent/emergency cases, passports are issued for short validity of six months or more but less than the normal validity of 10 years. In such cases the remaining validity of the passport could be granted without payment of additional fee as the fee for 10 years is collected at the time of issue of short validity passport.
1. Application in EAP-II form
2. 2 recent photos with white background and frontal view.(51mm*51mm).
3. Current Passport in original with photocopy (of first 2, last 2, address page if separate page, any other endorsement pages and visa page with additional booklets if any.
4. If the validity of the present passport expired more than 6 months ago, Sworn Affidavit for late submission, signed by applicant in the presence of Consular Officer is to be produced.
5. Personal Particulars Form (in Triplicate with pasted photos) of the existing passport, if the passport is issued by other PIAs.
6. Personal appearance of applicant is compulsory to establish physical identity.

CHANGE OF ADDRESS IN HAND WRITTEN PASSPORT (for making an Endorsement only).

1. Application in EAP-II form.

2. 2 recent photo with white background and frontal view (51mm*51mm).

3. Current Passport in original with photocopy (of first 2, last 2, address page if separate page, any other endorsement pages and visa page with additional booklets if any.

4. documentary evidence of change of address like Nativity Certificate in original issued by District Collector /Tehsildar /Commissioner/ Panchayat or copies of ration card, Election Commission ID card, water, electricity or telephone bill in applicant’s name and should be attested by the Home Department state government authorities.

5. Personal appearance of applicant is compulsory to establish physical identity.

CHANGE OF ADDRESS (involving issue of New Passport Booklet for printed passports)

1. Application in EAP-I form.
2. 5 recent photos with white background and frontal view.(51mm*51mm).
3. Current Passport in original with photocopy (of first 2, last 2, address page if separate page, any other endorsement pages and visa page with additional booklets if any.
4. Documentary evidence of change of address like Nativity Certificate in original issued by District Collector /Tehsildar /Commissioner/ Panchayat or copies of ration card, Election Commission ID card, Title deed, water, electricity or telephone bill in applicant’s name, and should be attested by the Home Department of the State Government concerned. For minor’s parent’s passport copy is accepted as a proof.

POLICE CLEARANCE CERTIFICATE (PCC)

1. Application in EAP-II form.
2. 2 recent photos with white background and frontal view.(51mm*51mm).
3. Current Passport in original with photocopy (of first 2, last 2, address page if separate page, any other endorsement pages and visa page with additional booklets if any.
4. Personal Particulars Form (in Triplicate with pasted photos) of the existing passport, if the passport is issued by other PIAs.
5. Copy of Latest Police Clearance Certificate in English (original to be shown) obtained from UAE Authorities.
6. Clearance from the Embassy.
7. Personal appearance of applicant is compulsory to establish physical identity.

On receipt of clearance from the concerned authorities in India for PCC for stay in India

Its advisable to obtained PCC before leaving the country

CANCELLATION OF ECR ENDORSEMENT

- for eligibility please see the attached page (In case the passport booklet does not have ECR stamp, the passport holder would be deemed to have been granted ECNR status)

1. Application in the format prescribed .

4. For canceling ECR Status in the case of dependents, (children/spouse) original and a photocopy of passport of the spouse/parent.

Who are eligible for cancellation of ECR Status:-

a. All holders of Diplomatic/official passports.
b. All Gazetted Government Servants.
c. All Income-Tax payers (including Agricultural Income-Tax Payees) in their individual capacity. Proof of assessment of Income-Tax and actual payment of Income Tax for last one year or copy of PAN card to be insisted upon, and not merely payment of advance tax. However, in most cases as an assessment order is not issued separately by the Income Tax department, Income Tax Returns which are stamped by Income Tax authority can be accepted.
d. All professional Degree holders, such as Doctors holding MBBS Degree or equivalent degree in AYURVED, or Homeopathy, accredited journalists, Engineers, Chartered Accountants, Cost Accountants, Lecturers, Teachers, scientists, Advocates, etc.
e. Spouses and dependent children of category of persons listed from (b) to (d).
f. All persons having educational qualification of matriculation and above.
g. Seamen who are in possession of CDC or Sea Cadets, Deck Cadets:
h. who have passed final examination of three year B.Sc. Nautical Sciences Course at T.S. Chanakya, Mumbai and
i. who have undergone three months pre-sea training at any of the Government approved Training Institutes such as T.S. CHANAKYA, T.S. REHMAN, T.S. JAWAHAR, MTI(SCI) and NIPM, CHENNAI after production of Identity cards issued by the Shipping Master, MUMBAI/KOLKATA/CHENNAI.
j. Persons holding permanent Immigration Visa, such as the visas of UK, USA and Australia.
k. Persons possessing two years diploma from any Institute recognized by the National Council of Vocational Training(NCVT) or State Council of Vocational Training (SCVT) or persons holding three years’ diploma/equivalent degree from institutions like polytechnics recognized by central/state Governments.
l. Nurses possessing qualifications recognized under the Indian Nursing Council Act, 1947.
m. All persons above the age of 50 years.
n. All persons who have been staying abroad for more than three years (the period of leave / stay in India to be deducted).
o. All children up to the age of 18 years of age. (At the time of re-issue at the age of 18 years, ECR stamping shall be done, if applicable).
 

NRI CERTIFICATE (FOR EDUCATIONAL PURPOSES ONLY)

1. Application in EAP II Form
2. 2 recent photo with white background and frontal view.(51mm*51mm)
3. Valid Passport in Original (which should cover the whole previous Financial Year, i.e. from 1st April, if not previous passport also) with photocopy of first, last, address page if separate page, any other endorsement pages and visa page with additional booklets if any.
4. Passports without residence visa may also be accepted in case the Applicants had not stayed in India for more than 182 during the previous Financial Year.
5. A declaration by the applicant to the effect that he has not stayed in India for more than 182 days during the PREVIOUS Financial Year (1ST April,….. to 31st March, ……).(Format enclosed)
6. If the applicant wants to have the name of his/her son/daughter mentioned in the NRI certificate, then the applicant should give a copy of Passport (1st and last pages) of the child. [The names of other relative of the applicant will not be mentioned in the certificate.]

CORRECTION OF DATE/PLACE OF BIRTH INVLOVING ISSUE OF NEW PASSPORT BOOKLET

1. Application in form EAP – I (External). All particulars in the application should tally with those in the present passport.
2. 5 recent photos with white background and frontal view.(51mm*51mm).
3. Current Passport in original with photocopy of page 1, 2, last two pages, any other endorsement pages and page containing valid visa and additional booklets, if any, should be submitted.
4. A passport expired more than six months year ago should be supported by a Sworn Affidavit explaining the circumstances and the reasons for late renewal. Sworn Affidavit is to be signed by the applicant in the presence of the Consular Officer.
5. In the case of passports issued by other Passport Issuing Authorities, Personal Particulars Form (in Triplicate with pasted photos).
6. A declaratory Court Order (in original with a copy) issued by a First Class Judicial Magistrate in India instructing the Embassy to correct Date/Place of Birth duly attested by the concerned state Home Department.

SURRENDER OF INDIAN PASSPORT

5. Declaration Form prescribed u/s 8 of the Citizenship Act, duly attested by the Embassy of India, Abu Dhabi (An Indian citizen with passport and valid visa to accompany the applicant for signing the Declaration Form).

7. Payment of penalty, if any, to be checked by the Embassy in terms of existing relevant instructions.
